ASSOCIATE MEMBER APPLICATION FORM

Thank you for considering Alpha Omega Phi Military Fraternity/Sorority, Inc.  We are dedicated to our four founding qualities:  Faith, Family, Country, and Integrity.  This application focuses on “Country” and “Integrity”.  We will need substantiating proof that the service member is / was in good standing with the military.  Below are the instructions for completing the application.   

 

1)  Completed Association Application (see below) 

 

2)  Proof of Military Service of Service Member / Merchant Mariner (black out SSN and pay information) You must submit in "Supporting Documentation" area in the application: 

 

      a.  Discharge or Retirement Orders, or DD214 (Member 4), or Merchant Mariner Credentials  

                                                      OR

      b.  Current LES (Active Service Members)

                                                              Applicant Information  

I. Payment Arrangements for Applicants

All Applicants are required to pay $500.00 initial Intake fee.
All MEMBERS are required to pay $105.00 membership yearly renewal fee.

Option 1
$500 – Paid in Full: This payment will be due by end of Week 1 of Intake.  

Option 2
$250.00 – Down Payment:This payment will be due three (3) days BEFORE the start of Intake.
$250.00 – Remaining Payment: This payment will be due by the end of Week 1 of intake process.

Once you select an option, you will be sent an invoice based on which option you selected for payment.


II. Refund Policy

Applicants will receive 50% of payment if withdrawing BEFORE the start of the intake line.  NO REFUNDS will be given after the start of the Intake line.  


III.  Applicant Expectations  

o Applicants must complete 6-week new members’ educational seminar.
o Applicants must complete 10 hours of community service.
o Applicants MUST attend assigned Alpha Omega Phi Commitment Ceremony.
